The purpose of this form is to provide me, as your professor, feedback on group projects. I need to have an idea of how much and how effectively each student contributed to the final outcome. Please be as candid as necessary. In cases where I discuss issues that you raise here, with other students in the class, I will be careful to conceal the identity of you as the critic and the exact wording of critical (positive or negative) comments you have made. I reserve the right to modify grades for individuals who have contributed substantially more or less than the norm on group projects.
